Is it possible to block or add a surcharge for payphones? This would be useful to soften the surcharge that's passed onto toll free number owners.

It's better to block it in provider side. you may use sparate 0800 for payphone use

In the US calls originated from payphones usually incur additional connection charges on top of the per minute toll free cost. These calls are usually identified by "info" digits transmitted to the switch together with the DID or calling phone number.

Can A2Billing recognize the call origination type and charge the call accordingly ?

if asterisk can recognize, yes a2B too. but most operators use separate did for pay phone (smart solution )
