The Postgres schema states the default for this field to be "YES", but on creation of a new SIP friend the qualify field is populated with "1".
This causes problems for SIP friends calls, and also DDI delivery as Asterisk believes all the SIP peers to be too lagged to receive calls.

Do all your problems go away if you set it to say 2000, yes or no ?

The field is limited to 3 digits, but after setting qualify to 999 all works as expected, yes.
