I haven't checked the source to see if it's definitely causing you problems, but the DTMF reader claims it's listening for 10 digits, then you dial 11 or 12 digits. Does changing interval_len_cardnumber to include 11 and 12 digits PINs change the outcome?

Again I'll say I've had no problem with Generate Customers on v1.3. Perhaps you should upgrade?

Added after 2 minutes:

I forgot to mention just yesterday I used Import Customers (on SVN branches/1.3 r392) to import 5,000 card numbers. The only problem I had was the import converts the whole line to lower case which caused the currency to not be recognised.

Taken with the previous result this is fairly conclusive. The question is why? Have you verified with the Admin interface that any of the cards were imported correctly? Is this simply a matter of hitting the 30 second default PHP maximum execution time and bailing out before finishing the import, or reaching the maximum file import size (512KiB by default I believe)? What is the result of the import itself? (something like "Congratulations, 5,000 cards were imported"?)

If the cards are present in the admin interface then surely it's just a problem with the cards being 'activated' or perhaps the start/expiry dates need to be set to sane values.
