When you run A*, make sure you have a verbosity level defined. This also needs to be configured for A2B (config screen)
Make sure also the simulator can rate the call - that's a start ! also, check that the rates/ratecards/call plan are linked together (specially, dont forget to add the rates in the ratecards)
re your questions q1. in extensions.conf, there are the few lines recommended in the install wiki to add (deadagi call) q2. it depends on the format your provider accepts, and how your customer dial - you may need to play at the trunk definition level to remove some digits, and add others, so the call goes through.
at this stage, looks like you're almost there, and I'd suspect a lot of help can be found in the V1.3 section as well - hopefully, while a number of great improvements have been made, a lot is still common between the two versions,
