uploaded rate card, all destinations show up in call plan with calling rates.
when trying to make call using that call plan, it always busy
and simulator for that plan does not work.
any idea??

Try to look in log files:

/var/log/asterisk/
and
/var/log/a2billing/a2billing_agi.log

You can post the logs here, so i will help you.
