rates in cdr

when rates get changed within the same ratecard, then the new rate is shown in cdr, instead of the rate that have been valid when the call was made. its better to save the rates, that were valid when the call was made, into the cdr.

The "rate id" is stored in the CDR for reference. The reports and invoicing look at "session bill" .
