The issue is when "Enable Expire: EXPIRE DAYS SINCE FIRST USE" is selected.
If I select that and enter "Expiry days: 30", for example, I am returned an error.

This is clearly a a bug. If I select N days to expire, then why am I the one needing to calculate the expiration date and enter it in the "Expiry Date" date field?

The form should allow a user to not have an expiration date in the field if the Enable Expired field is either "Expire Days since first use" or "Expire Days Since Creation". The db defaults the expiration date to "0000-00-00 00:00:00". From a user interaction point of view, it is not obvious how/why the expiration date field has to have a value in the two cases mentioned.

The db may default, but the form does not, thus causing this bug.

I see a date some 15 years or so in the future.

Right, I was agreeing with you. The form should allow null input for fields that are not required if the database does so as well.
