Looks like you are using V1.1 Chameleon version. Can you please share with us portions of your a2billing.conf & related contexts in your extension.conf file with the confidential data sensored out?

I'm pulling my hair trying to get this rolling - even though I have confirmed that all pre-requisites have been installed.
